I could go outside with a nice coat on, no pants, no gloves, nor udnerwear, and still be marginally warm.
Snipers would have a bitch of a time.
Fistfighting would now need at least a rudimentary knowledge of mathematics.

Everyday strangers can only say a few lines of dialog.
You can run from New York to LA, with boredom as the only possible drawback.
You can carry 4 sets of armor, 6 large weapons, over a dozen potions, and still be free of exhaustion.
Almost all "people" have some kind of quest or mission for you. "I lost my purse to a horde of orcs living in a collosal cave to the north. Can you get it for me?"
Every now and then you come by someone who looks exactly like you.
When fighting someone, you take turns, just to be fair.

Potions instantly heal you and stop all bleeding in under a second
No one else ever seem to sleep,they just wander the streets going back and forth doing nothing
Everyone else seems to look exactly alike
If you die you can load a new game and start over

You can have multiple personas and be non-schizophrenic.
When you die, you can respawn at the cost of 10% gold and 50% XP.
The more experienced you are, the higher health you have, to the point where not even an axe to the head will kill you.
You won't ever have to take a poo.

Leaving leaving/entering your home, and crossing state lines would bring up a loading screen.
New players (i.e. babies) would often be killed by swamp rats and giant butterflies. This would be ok though, since they could just respawn.
People would sit in the maternity ward, waiting to kill people who are respawning.
You could carry 70 times your own body wait.
There would be no fat chicks.
Old people would be super powerful instead of super bed-ridden.
We'd get bored with this life after 6 months or so. We would then switch to a new life and make fun of the people still living in the life that we just left.
Doctors would be useless.
When priests tell people of Jesus's miracle of resurection the people would react with "Pfff, STFU n00b! I c4n d0 th4t t0. I pwn j3su5!!!!11".
For some reason, small animals would carry pole axes and other large weapons. This would not be questioned, but rather accepted as a fact of life.
The concept of air vehicles which can roll more than 90 degrees escapes modern day engineering technology.
Missles capable of destroying a vehicle in 3 shots will only slightly damage a human with 3 shots directly to the face. Again, this will just be accepted as a fact of life.
When fighting, we will take turns using our weapon. If one combatant misses, they will wait for the other combatant to attack before they attempt to attack again.

Firefights in the streets whould not be that uncommon, in fact its very likely that most citizens walking down the street whouldnt even stop to watch, by comparrison they most likely whouldnt run away either.

Towns would be occasionally invaded by many large powerful monsters, but the town guards just sit there with thier pikes, and watch, while a group of adventurers happily slays them.
