What system are you running? I've switched mine over to high settings tonight to see how well it runs, though I'm using laptop built to run CAD programs, so I've got an i7-quad at 2.2GHz, 8GB RAM, a fairly fast hard drive with loads of space left and an Nvidia Quadro 2000M. No frame rate issues, any sort of stuttering or tearing, sounds working good, and my internet connection where I normally play is pretty slow, only maxing out at 600 KB/s, though I have used one at 12 MB/s to run it before.
