Here's a point which I don't think has been touched upon; when you die, what do you actually look like?

I know there are coffins, but when you die, can you choose to stick around and be revived, or go straight to the spawn queue?

If so then there must be some way to distinguish between the two for medics' sake. i.e. when you die there is a corpse, and if you choose to go straight to the queue you turn into a coffin.

Hmm.
