After playing doom3 i can safelt say that anything is possible graphics wise.
Check out the AA between the armoured breast plate and his neck, a dead give away to a 3d model. Surely if it were rendered this would be smoothed considerably? I would think that the third image has been touched up, but might possibly be an actual model after reviewing the new unreal engine. One day we wont be able to tell the difference.
