I would suspect that is more attributed to lag and zone issues.

You could be running around or in a vehical and just start taking damage for no apparent reason. It won't stop either, it isn't that much damage but it's annoying.

Thats simple.

Sometimes, when many people are in one place for a while, say, a vehicle pad, the game, for some reason, decides to make a copy of you.

See those ghosts that sometimes at vpads that sometimes wont die? Yeah, they are still in the vehicles they just got.

Unload a few decis into that ghost, and BOOM, <insert wall hacker comment here>

Sounds like lag to me. Someone shoots at you and nothing happens. You don't know it because you don't get any damage. Then the damage hits later on. I get it all the time in tower battles. Step around the corner and fire a few shots. Duck back down the hallway away from any enemies just to start taking damage well behind cover.

I've actually died in the basement of a tower after being shot up on the first floor. It happens now and again.
